We take a medical history on our clients. We create a personal history with them.

SMHC provides a wide range of high quality primary medical, disease prevention, dental and podiatry care. Pre-natal and well-baby care, patient education and outreach are also available.

Our services are tailored to meet the special health needs of our community. Although specialty care is primarily provided by referral, we provide expanded services on-site for HIV/AIDS, gastrointestinal disorders and diabetes. To address problems associated with pain medication management, SMHC is implementing a chronic pain management program, with a special focus on dual-diagnosed clients. And, SMHC has a satellite clinic to serve the specific needs of our seniors.

Yet what truly sets us apart is how we deliver our services. Many of our clients have had difficulty accessing quality healthcare, and come to SMHC with multiple health problems. At SMHC, we take the time to listen to our clients and understand their values, beliefs and customs. By establishing a relationship with our clients, we help overcome the barriers that have prevented them from receiving quality healthcare.

Our approach includes treating the whole individual, not just their physical complaints. In addition to providing medical care, we refer patients to support and enabling services for substance abuse, mental health and social services.

 

Building on our success:
In Spring 2008, we’re expanding to our new Westbrook Plaza building. This joint project with Mercy Housing California will include 48 units of affordable housing and a new health center.

Bruce's story:

Bruce has been coming to SMHC for 18 years. He represents SMHC at the San Francisco Clinic Consortium and has traveled to Washington DC to advocate for community health funding. He would like to start his own greeting card company, “I’d like to be able to employ the homeless. It’s not about being financially secure, but making sure others benefit. That’s what I’d like to leave behind.”
